A 14-month-old boy had high fever and cough for 3 days prior to admission.
Significant PE included:
Temp 39 - 40.5 C, RR 44/min, HR 140/min. BP 90/50 mmHg.
Decreased breath sound at RL and bronchial BS at RUL.
 
Significant initial laboratory findings included:

CBC: Hemoglobin: 9.0 g/dl, hematocrit: 30 % , platelets: 476,000. WBC: 34,300
(Differential: 81% polys, 19% lympphocytes).
CXR: as in Figure 1
