CC:  Fever for 9 days.
PH
: Not significant.
PI: He was well until 9 days ago he started having fever. For the last 5 days, the mother noticed that his fever was high and intermittent. She denied any other symptom.
 
PE: Body temp was 39.8 C. Other vital signs were within normal limits.
Skin: A skin lesion was found at right axilla as in Fig 1 and 2.
Abdomen: Liver was palpable 4 cm. below RCM, normal to firm in consistensy, spleen as palpable 3 cm. below LCM, normal to firm in consistensy
Other systems: were within normal limits.

Investigation: CBC:   Hb 9.9 mg, Hct 36.2%, WBC 9,640 (N 30%, M 7%, L 63%), platelet count 84,600
